Child Enrollment Forms
All enrollment forms plus a copy of your child’s certified birth certificate and physical (on IL DCFS form and administered in the past 6 months) constitutes a complete child enrollment file and should be submitted a minimum of 3 business days prior to your child’s start date. To initiate the enrollment process, please complete the Application For Child Care, Fee Agreement and Tuition Express forms. Once these initial forms are received along with your child’s enrollment deposit, all additional enrollment forms will be provided to complete your child’s registration process.
